There are three ways to get your tax forms.
- The simplest way is to download the form you want from the Revenue website at www.revenue.ie is by going to the Forms link on the left hand side of the page. Please note that a downloaded Form 11 is not acceptable as a return for your Income Tax. You will need a Form 11 posted direct from Revenue, or send the return in online on ROS.
- Call in to your local tax office and collect the form you want. The address will be on your tax correspondence or at https://www.revenue.ie/wiclo/details.jsp (you will need your PPSN number). Another way to find out where your local office is, is by looking at the Revenue website at www.revenue.ie. Go to the Contact Details link on the left hand side of the page and then click on the relevant part of the map of Ireland.
- Phone up your local tax office and ask them to post the form to you. The phone number is in your local phone book, or on the Revenue website at www.revenue.ie Go to the Contact Details link on the left hand side of the page and then click on the relevant part of the map of Ireland.
